Is the Mario Foreskin Comic Real?

No it is not real, it's a fake. It's an edit. There's your simple answer, but it's also a really good edit. Take a look at how this appears to be a picture of a printed artifact (like the real comic) in how the camera captured a tilted photo and the top of the letters get a little blurry. That's either very intelligent photoshop, or somebody printed out their edit and then took a picture. Either way, kudos. It probably fooled a lot of people. And hopefully it convinced many others to wash their foreskin.

What's the origin of the Mario "Don't forget to wash your foreskin" comic?

It's actually a Revlon ad for kid's Nintendo/Mario themed shampoos/bubble bath formula. They had Mario and Princess Peach options. In the original ad/comic, Mario says "Our bubble baths are a real splash too!". And though peach will tell you that you're going to smell "fruity fresh", you won't find any mentions of foreskins.
